Head and neck tumours are among the most complex cancers in terms of treatment and impact on quality of life. This disease can affect the oral cavity, larynx, pharynx, salivary glands, nasopharynx and adjacent structures (directly affecting speech, swallowing, breathing and the patient’s appearance). That is why head and neck cancer treatments require a particularly precise, personalized and multidisciplinary approach.

Modern advanced head and neck oncology combines classical methods with innovative regional technologies, especially in cases of advanced or recurrent disease. For many patients, head and neck cancer treatment abroad (in particular treatment in German specialized centers) is the optimal solution.
Early Symptoms and the Importance of Timely Diagnosis
One of the reasons for the difficult course of the disease is late diagnosis. Early symptoms of head and neck cancer include:
- Persistent sore throat
- Difficulty swallowing
- Hoarseness
- Non-healing mouth ulcers
- Swelling in the neck
Early detection significantly increases the effectiveness of treatment and expands the choice of therapeutic strategies.
Standard Treatment Methods: Proven but Limited Basis
Traditional treatment for head and neck tumours includes surgery, radiotherapy and systemic chemotherapy.
- Surgical treatment remains the main method for early-stage tumours of the oral cavity, larynx and salivary glands. It involves removal of the primary tumour and regional lymph nodes. However for large or difficult-to-locate tumours, surgery can be extremely traumatic. Even after reconstruction it is not always possible to fully restore function (cosmetic defects often lead to psychological disorders, social isolation and depression).
- Radiation therapy (in particular IMRT) allows for a targeted effect on the tumour preserving healthy tissues. At the same time, in advanced forms its effectiveness decreases and side effects (damage to the salivary glands, larynx and mucous membranes) can be long-lasting and significantly worsen the quality of life.
- Systemic chemotherapy is used for metastatic forms, but it affects the entire body. High toxicity, immune suppression, nausea and exhaustion often limit the use of effective chemo doses which negatively affect treatment outcomes.
Regional Intra-arterial Chemotherapy
Among innovative head and neck cancer therapies intra-arterial regional chemotherapy (RCT) occupies a special place. This method is fundamentally different from standard systemic chemotherapy as it enables the delivery of ultra-high concentrations of drugs directly into the vessels, that supply the tumour.
The basis of the method is the catheterization of the carotid or other feeding arteries, followed by the local administration of cytostatics. In more complex cases, isolated perfusion is used (in particular isolated thoracic perfusion (ITP-F), in which special balloon catheters temporarily block the aorta and vena cava. This creates a closed circulation in the tumour area, “holding” the chemotherapy drugs in the affected area for 10–15 minutes.
After the infusion is completed, chemofiltration is performed which clears the systemic bloodstream of drug residues (significantly reducing overall toxicity).
Key Benefits of Regional Chemotherapy
RCT is considered one of the most effective head and neck cancer treatments for certain patient groups especially for locally advanced and recurrent tumours. Its advantages include:
- Ultra-high local drug concentration in the tumour
- Minimal systemic toxicity
- Preservation of speech, swallowing and appearance
- Significantly lower need for tracheostomy or tube feeding
- Rapid clinical effect (sometimes after the first cycle)
- The possibility of multiple repetitions without additional operations
Quality of life preservation makes this approach extremely important in the treatment of head and neck tumours.
